I'm not so sure. Transsexualism is definitely, doubtlessly, and quite obviously a fetishistic disorder, but many fetishes can be explained in either evolutionary or cultural contexts. Foot fetishism for example, disgusting as it may be, seems to be the product of first world cultures in which one simply very rarely sees the bare feet of others. This seems to grant feet the sense of being forbidden flesh, which can, in some people, create an allure.
Christ, I never imagined I'd mount a logical defense for foot fetishism. The decay of society has gotten to me.

Aging up a character doesn't change the fact that canonically the character is a child and thus stuff like that is disallowed as some type of child porn. The rule also has to do smtn with peoples' styles so sometimes when a character looks like a kid nothing can be done to it because it can be "just muh style", unless someone can prove the character is underage.
What comes to oppai loli, it is pretty much it's own genre but it is more often used as a label because of the artist's style than because of kid characters having boobs.

This is actually very curious to me, since I trust on studies saying it's most likely developmental issue (the body and brain of a fetus get different "gender signals" and either starts developing wrong) similar to some intersexuals (people who're born with both sexes). I know a few trannies personally and the females transitioning to males have not shown any sign about fetishizing it and they have generally low interest in sex, partly because they don't like the body.
However almost all of the males transitioning to men who've been in publicity obviously have fetishes about being women/little girls getting raped/molested/fucked by daddy and so on. Well, when people were still "required" to wear enough clothing to hide everything but their head, hands and little bit of legs (just above ankles, sometimes not even that), people did associate the "hidden" body parts more heavily with sexuality. Women showing off their shoulders, ankles and feet were seen as frivolous sluts and prostitutes.

So it is possible, although the more likely option is that feet and ankles are some of the most sensitive body parts. Some can get orgasms or experience other types of pleasure from having their feet massaged and so, and it's not rare either. The most relevant study about existence and proving transsexuality is actually about MRI brain scans showing trans people have brain more closely resembling the brain of the opposite sex.
But I agree when it comes to the operations. Sex change is more often than not an unsuccessful attempt with curing the psychological problems and they tend to come back even worse afterwards. Even the patients who want to go that far are really unstable in comparison to those who're content with just passing as their preferred gender.
Hormone replacement therapy then again does help the patients and possible unwanted side effects are generally easy to deal with.
All in all, diagnosis for being trans is too easy to get almost anywhere when there is issues which cause symptoms resembling it.

Transsexuality is comorbid with many other conditions, such as chronic anxiety, major depression, and bipolar disorder.
I do not deny this. What I am unsure of is whether transsexuality is a symptom or a disorder unto itself.
